Checklist: 

• Authentication Application (make sure to sign on the back page for 2 signature spots, the signature should be the same as your passport/ID) 
• Passport and/or ID, color, with picture ID, front and back (both parties)
• Relevant documents, scanned copies
• Document to be authenticated: Notarized signature if needed 

Please scan/take a picture (no reflection, with all 4 corners, clear and focused) of all the materials to info@SDTranslationCenter.com before you send the documents.
